    This applicattion form is giving us some problems. People is trying to enter but they can't, there is a blank page or an anti-virus alert. Thank you very much.

    Hi,

    It is working fine on my end, I haven't experience any Anti Virus Alert accessing your form.

    Would you please ask them to provide a screenshot so we can check and possibly resolved that issue on our end?

    Hi Kazi Shah,

    At the bottom of the form you have a link "Forgot Password". You need to click on that and follow the instructions to reset your password.
