Form returns blank page after submission

Can you help me figure this out? When I submit this form, upon submission, it gives me a blank page (same URL, but absolutely blank... no HTML or code whatsoever).

Please help me... I've submitted a case before about this, but the problem seemed to somehow resolve itself. Unfortunately, it has returned to this state once again and I'm in trouble with the boss. :-(

By the way, when the problem seemed to resolve itself before, it seemed to start working after I tried to re-fill and re-submit the form info several times. I haven't dont that this time, because I want the broken condition to be there so you can see it not working.

Sorry to say, but we can't help you to solve the issue. Upon checking your website, I found out that the form which embedded in your website is not JotForm form. You may contact Google instead about this issue since your form is created using Google form builder.
