"This form is currently unavailable!" Why, What causes this, How do I correct the condition

Since under "More" it appears to be possible to Disable, but it is not in that state at this time.  This condition has been in affect for three days.

I need to know what caused it and how to fix it.

The reason as to why you have this issus is that you had limits set on the form as shown in the screenshot below:

I have removed the limit to avoid having the form disabled again.
