Please follow this guide http://www.jotform.com/help/14-Adding-Form-to-Blogger and re-embed the iframe to your website. I have reduced your SSL submission and activated your account. We can only allow this once if your account exceeds SSL limit the next time it's either you upgrade or wait for the counter to reset which happens every 1st of the month.

I have set your SSL counter to 5 try re-embedding the form and let us know if this still does not work. I just did a test submission and the SSL no longer counts as a regular submission.
