URL improperly showing up on mobile site

    The link I added "fill out online here" opens the correct URL. For some reason when you open the mobile site your URL is there in blue. I didn't put it there. How do I get rid of it? On my admin site this blue URL is not there, just pops up on the public site. The process worked fine on my flash site, just not the mobile. Any suggestions? If you click on the blue URL you just get an error message.

    It seems that the form on your site is somehow embedded within a link. I think that this is not a correct way to embed the form:
