my form don't work correctly

I have made two forms with diferent pages. When I click on the next button the form don't go to the next page. What can I do?. The forms had included in web with Adobe Muse program. The url's are:

Upon testing this I can see on your site it is not working so I tested the standalone version of your form where it did work. Based on the current situation I think that there is likely a conflict with your website coding and the form coding that is causing this issue therefore I would suggest trying out an iFrame instead since this usually halts that conflict so that your form can work properly.

Also, just to clarify this was the case for both forms. Try replacing the code with the iframe for one and test it again just to see if that helps.
