When someone follows the link to my form it is filled with the last person's information?

when someone follows the link to my form it is filled with the last person's information? I want each person to get a new form.  I have a first form that enters the person's name and email - once they submit that they are directed to the actual survey by a link - but the last person's information is in the fields?  I have page breaks, and I thought it was set up to allow users to go back to edit their forms, but I want new users to have a new clear form? I have provided the link to the first form - which redirects to the second and that's where the info from the last person generally displays,  Please help?

I would like to recommend you to redo your session links on the Autoresponder and Thank You page.
