Please wait displayed and hangs after submitting the form

Also, when I tried to test out the form after putting in the embed code, Instead of sending, the Please Wait box just stays up and I haven't received the form in my inbox. 

It seems the issue is with browser cache. I request you to clear the browser cache by following the steps provided in http://www.wikihow.com/Clear-Your-Browser's-Cache
