Can I create a form like the one on www.victorymmatx.com (HOME PAGE)

    I use the forms on adobe muse but i get a lot of spam mail. I was wondering if y'all offer a design for the form like the one i have on the home page on victorymmatx.com 

     

    Basically I just want to create a form with JotForm that looks almost identical to the one on victorymmatx.com (probably going to add the capcha option)

    Yes it is possible to create similar form in JotForm.
