Form Unavailable

    Here is the link (http://www.ctwoodlands.org/JCA_2014)  on my organization's web page that has the 3 forms I'm using, the one that is not working is the link entitled "registration and waiver form." You'll see the other 2 Jot Forms are working fine.  I've tried copying and pasting several times as an embedded form.  The form worked fine when I first embedded it, then some time last week while I was on vacation, the form became "unavailable" when the link clicked on, but the other 2 still work. I've copied the link several ways: the direct embed form, from the Drupal button, then form the source code, and also from the iframe.  Orginally I had copied the code as a secured form directly from the embed wizard for all 3 forms.  When clicking on the Drupal button, it didn't work originally, so I just copied from wizard button directly.

    Hi,

    I checked the form in question and it appears you have set a limit of 11 submissions for that form.  Once that point was reached, the form was automatically disabled.  If you would like to enable it again, click on the preferences tab in the form builder while editing this form and click on form limits:

    From there, change the number of submissions you would like to allow for the form.  Once that is done, click on general settings and set the form back to enabled:
