Form is missing items when submitted

I built an order form from one of your order form templates, I added in the photos, but I also needed to add text beside the photo ( I don't know why but it will only let me add text as an html format, it wont just let me add text). Everything shows up on the order form as it should, but when it gets sent to my email, some of the form is missing.

The form template that I used;

"Orderform" - ( van nieuw form )

This is what it looks like when I am working on it; (with the name, email, phone number, photos and text added )

This is what it looks like in the email when the form is submitted;

The fields for Name, Email and Phone Number are not there. So we don't even know who placed the order.

The description of the item is also not there, so we are left with matching up the line number of the prices  to our product list.

Can you please advise us as to what might be going wrong ?

I see that some fields are removed from the email notifier in which you are receiving without Name and email address.

As I checked your form, the product images or description are not linked to pricing or weight fields. They are treated as independent entities and hence they're not included in notifiers. This is by its behaviour.

To best resolve the issue, you may consider integrating the payment tools like purchase order that gives you more flexibility in managing the products with variety of options.
