I'm attempting to setup a conditional on a form of ours. If i setup a conditional for a single checkbox, does the fulfillment if it fail fall back on the form notification that is default? It doesn't appear to be sending properly when we test it.

Hi,
One of your form notification uses two notifications at a single condition. I would suggest combining the emails just like your 2nd condition and separate it with comma.
Example:
Also, you don't seem to have conditions if both options are selected which means nothing will be sent. Have you considered cases like that? Since you are using checkbox, it is possible to do multiple selection (Digital & Collateral). You should probably create a separate condition that would serve as the default notification once any of your first two conditions aren't met.
Example:
If "Required Creative Team" is equal to Digital
If "Required Creative Team" is equal to Collateral
If ALL of these rules matched, Send Notification to ....

Or you can simply create a new notification without any conditions.
