How can I create the following thanksgiving registration form?

Hi, what I am looking to do is create a form that would ask the user to input their name, number of guests and item they are bringing for thanksgiving. I would also like to have on the page (before they enter their info) a list of everyone that has submitted their information as well as what item they are bringing to avoid duplicate items.  Thanks!

You can take a look at this test form: http://form.jotformpro.com/form/42866785937981 that I've created using one of the available templates from the event registration category section to get a better idea.

About the list you want to show, I think that you can accomplish this by using a Data Grid widget and let your submitters fill the items that they will bring there.
