Conditional Fields stopped working on Oct. 16

I had a successful form (which contained conditional fields) for several months. On Oct. 16 2014, all of a sudden the conditional fields (that I had set up to hide or show fields based on a response to the first question), just no longer worked properly. When I look at the rules in the edit form page, everything looks the same. When I actually view the form in a browser, only condition 1 is upheld-conditions 2-4 are not upheld. Any suggestions for what may be going on?

As far as I know, we had a temporary problem with conditions on iFrame embedded forms for several hours, and it was fixed by our Development Team.
