I created order form but doesn't display correctly

I created a order form. Copied it to my Weebly hosted website page.  published it and the total cost US Dollors and the submit button displays for a quick moment and then disappears. Everything looks good when I edit it in Weebly's software. Not sure what the issue might be. Had a similar problem with another form and never got it to work. Any thoughts?

To visit the website problem page go to www.14thtransbnamgs.org/. Select the "PX work" tab. Scroll up to the Reliable s hat order and select the "HERE" on the order here to select the copied Jotform.
