Why all of a sudden is my "Next" button not working?

For some reason my form has stopped working. Up until today I have had no issues but for whatever reason the "Next" button on my form no longer seems to work.

Folks can enter in all the information and make their selections from the radio buttons but when it comes time to hit "next" the form will not let them proceed.

I myself have tried it a number of times on 3 different browsers with no luck. I have even disabled and reenabled the form on jotform as well as resetting the code on my website.

Any insight or help would be greatly appreciated.

I was NOT able to reproduce the reported issue, as I was able to use the Next button and even successfully submitted your jotform http://www.jotform.us/form/30725398831157 when I test submitted it on your website.

If the issue persist, please try also re-embedding the jotform using its iframe embed code instead.
