I need to somehow get the below matrix within a jotform.
The way I figure it there are three options:
1. Ideally, have a matrix with this information where a user could click a cell, checkbox or radio button next to each datapoint within the matrix. Right now I can't figure out how to add text next to a radio button or check box.
2. Have a static grid with the jotform showing the above information, and separately have drop down boxes below the grid to indicate the chosen options. I can't find a static grid on jotform; they are all blank grids that allow user input.3. Embed a pdf, jpeg or some other visual image of the matrix -

I've opened up a new thread for your question #2 and 3, this is for us to further assist you by focusing on one topic per thread.
Regarding your first question, unfortunately, we can't add label text on each options of the matrix field. And the configurable list widget seems to be not the right solution for this. I do have some workaround in mind and I'm still working on it, but basically I'll just use radio groups instead of matrix to have some kind of table-like listing that you have above.
