I have read the tutorial on how to make an approval form going to one supervisor, but what if I have 5 different managers all with different employees reporting to them?  Is it possible to make a condition so the form is sent to the appropriate manager?

Yes, you can use conditional logic to send different email notifications in your form. Please check this guide: http://www.jotform.com/help/167-How-to-send-to-a-specific-email-address-based-on-a-selection

In your scenario, you could use a field for each supervisor to submit their approval and use the same field to trigger a notification for the next manager, in case a supervisor approves the form after the other. You can also use the same guide in case you want to control to which supervisor the form needs to be sent for approval. In this case, you'll need to add a field in your form that will be used to select to which manager the approval request should be sent.

    Hope this makes sense.
