Why is my form showing that its been disabled?

When I try to preview my form, I receive an error message that says "Form not found. Form is disabled." I've checked the settings and it says it's enabled, but will not let me preview it or grab the link to send it to recipients. How can I troubleshoot this?

I see you are wondering what happened to your jotform after trying to preview it.

I've inspected your account where I found this happened because you were suspended by our system after importing a form which is requesting Login Information such as usernames and passwords. This is not allowed because it is considered phishing and violates our terms of usage.

I have reactivated your account must request that you remove "[Imported Form] 43504733027146" to comply and avoid being suspended again in the future.
