Using 1 form on multiple pages, getting referrer title from each page

I have 1 awesome form template that I would like to use on a lot of pages without having to clone the form. I can include the embed code in my PHP page template, but I won't be able to tell on which specific page the form was submitted. Is there a way to use my site PHP to add the specific page title or URL to the form on submit?
