Auto suspended form due to violation of Terms

Below is the embedded link for the form I have created and every time the link is clicked the message says the form is disabled or un available.

Your account and form was auto suspended by the phishing filter because of the suspected phishing form you have http://www.jotform.co/form/50218239081854

Creating such form is in violation of JotForm Terms of Use.

In the Phishing clause...

Phishing You agree that Company may terminate your JotForm Service immediately If a form is created or JotForm is found to be used to: Obtain credit cards sensitive information, get social security numbers, show content with child pornography. Or if a form was made to steal user login credentials. All these activities are considered as Phishing and any account along with the guilty forms will be suspended immediately 

You either delete this form or update it by removing the input that ask for CC names, number, signature before we can reinstate the account.

You should instead use the official Payment tools available to JotForm as they are more secure.

Thank you for your help Jot support. I apologise for the honest mistake. New user and thought you can create your own more suited to the users needs. Can you please reinstate the account.

Upon checking your account, I found that it is already active and your account is currently in good shape. You seems to have also deleted the suspected form as well.
