What did I do wrong?

I created a form and the confirmation email I received looks nothing like the form I created.  The verbage is different.  The layout is different. I linked the form through my Facebook page, as I really like that feature, but it's nothing at all that I wanted linked.

Can you help me figure out where I went wrong with this?
