Adding form into volusion

I created a form and would like to add it to the website that I am building with volusion. But having trouble finding the right code to insert into my website.. how and where should i find the code? thank you very much for your help. I tried inserting the code but all that comes up is the code ( attached screenshot)

Have you tried pasting the embed code while on HTML mode or source code view on your post editor? Is that something available on volusion? I think it's pretty similar when you embed a youtube video using its iframe embed code.
