The Facebook does not show tabs on the mobile site. However, we can use special URL to redirect the user to the desktop version of the Facebook.
For example, if the standard form tab link:
https://www.facebook.com/web-page-name/app_684336108254110
You can change it to the following format:
https://www.facebook.com/web-page-name?sk=app_684336108254110&ref=s
Notice added ?sk= instead of / and also added &ref=s parameter to prevent mobile site redirect.
That link should work on all devices.
Unfortunately, I was not able to open your facebook link to provide you with the exact URL. If you need any further assistance, please let us know.

so are you saying that someone with an iPhone could actually fill out my form from their iPhone if I do as you said..

It works (no redirect to https://m.facebook.com) on my test page:
https://www.facebook.com/294069087374653?sk=app_190322544333196&ref=s
But, for some reason, it does not work with your page link:
Looks like you are using the 'Static HTML: iframe tabs' App.
Have you tried their 'Enable tab on mobile devices' option? Does it work on your Facebook page?
