Email notifications are not being delivered to my email address

    Also, I am not receiving email notifications for this form. They show on JotForm but no emails. Perhaps there is something fundamentally wrong with the form design.

    I took a look at your jotform and I see that there are no emails set for it.

    Please follow these steps to add the email notifier to your jotform which will allow you to receive emails on notifications: Setting Up Email Notifications

    I'm having the same problem.  I am not receiving the email notifications from http://www.johnromano.com/vacation/

    However, they are being saved on jotform admin area.
