Jotform not showing up after adding it to Facebook tab

But another problem occured. The form does not show up but as in the below capture.

Based on the image that you have posted the URL of the jotform was not possible to open.

Now I presume that the issue is with the link of the jotform that was used, but would just like to check.

Could you please give us the link to the jotform itself (that you have used) and we will see what might be the cause of it.
