Cannot import form from website

I am not able to import this form from a website. Cant i upload my own source code for the form? Or could you port the following code for me

Unfortunately our import tool does cannot import all forms, as forms can use all kinds of different techonologies.

This form is one of those. It is necessary to create the form from scratch.
