Requesting to change user name

I would like to change my username to represent the school organization I represent. I am trying to create a student application. I noticed when I tried to make a custom URL it has my username in it (www.jotform.us/valangie/mentorship_application). I'd like the URL to have the name of my organization which is why I want to change my username to RU_IPhO.
