Form does not load, both on Preview mode and in standalone version

I have completed the work on my form and when I click preview the screen is blank.  I did not change anything except the text area where I was asked to update the informtion.  Can you please help

I tested and unfortunately there was a bug with the page breaks. I created a clone and removed them and the form start appearing.

By removing the page breaks and adding them again the situation should be solved.
