My form is displayed wrong when I embed into my site

The phone number boxes are stretched to the max, as is all fields.

When I preview the form, it looks fine.  But NOT live!

The iFrame suggestion, which I thought was a good one, did NOT work.  In fact, it looks worse.

Thoughts???

It is quite odd, I checked your form settings and they are set to default, this usually happens when you enable the form responsiveness, but it is not the case in your form. 

I would recommend you to clear your forms cache, then try re-embedding the iFrame code without the script part:
