Zip postal code is being returned as city in test emails

Why is the zip postal code coming up in a test email as my city and not Alpha/numeric values? 

I am not a programmer and should not have to learn scripting language to fix what should be set and working in the form boxes values.

Could you please tell us what is the form that has this issue and if the same issue happens when you submit the form, or only while clicking on test email button?
