Can I create Forms in following formats: Web, PDF, and Embed?

Also, does JotForm provide an email receipt of submitted material to the respondents as well as the host/surveyor?

We want the person submitting the information to see what was submitted as a receipt. As the administrator, we want to make sure we receive an email with the data submitted as well.

Both of which will contain the data entered into the form by default and can be altered to your liking.
