My form isn't lining up properly on the left.

Can you please look at my form and let me know why it isn't alining properly. When I page through it the various fields are not in a line on the left and it looks very sloppy. Can you please help? There are some areas (like the title) that I want centered but the items that are meant to be left aligned are scattered. Look on last page at q10 in contract the rest of questions. 

I've tested this in Google Chrome and Safari with the same issue on both browsers.

Page 1: I think it is fine.

Page 2: It seems that the first 2 items under the heading are a tad to the right. Almost like there is a space in front of the sentence (I checked, there isn't. lol)

I also noticed that there was a Payment tool on the form that currently is not configured for use.

I suggest removing it if not intended to be on the form

I haven't activated the payment tool but it will be prior to opening to clients.
