Embed Form not working on Tumblr page

I've been trying to add a contact form on one of my Tumblr site's subpages, but it doesn't seem to work. I've also tried using iFrame for it.

Is there anything wrong with the script?

There are 2 methods to add the form to Tumblr website.

1. using script embed code

OR

2. using link

Can you please try also either of the 2 method. 

Let us know if none of the 2 method works.
