Is this posible to create a table like form?

Trying to create a very custom form...

Custom backgrounds in different sections.

Multiple fields on one line.

Ability to total up cost of several lines.

Online signature

I am looking for the absolute best solution to our needs.

Possibly a PDF? Not sure what to use and your help is appreciated.

Prefer load to Google Drive when submitted along with copy to customer.

Very important that this form look VERY similar if not identical to the attached screenshot.

One page for easy printing when submitted.

The form calculations will not work on a PDF form, so it is better to stick with a web form.

The problem is that we do not have any form templates or layouts similar to the one on your screenshot. There is no any easy way to create such a table-like form using our form builder. 

We provide a full source code of the form, so in theory it is possibly to completely redesign the form with custom HTML and CSS in order to get mentioned layout. Note, that in this case, you will need to host modified form code on your web site.

How to get the Full Source Code of your Form

Although, we do not provide design services. You might consider hiring a Front-End Developer or HTML/CSS specialist to modify the form code per your requirements (for example there is a Freelancer Marketplace site - https://www.odesk.com).
