Error - Test embed code

When performed the test embed code form part of the page disappears .
Can anyone help me ?

It seems like the images you have here were broken during the process when you were uploading them. Could you try re-uploading them again?

Regarding the issue, could you try re-embedding using the iFrame embed code instead? Here's a guide on that: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Also, if the website is live, you can share us that web page link to where the form is embedded and having problems so that we can further check. We'll wait for your response.
