Video not showing when form is embedded on Dreamweaver?

2/. when i embed the code across into Dreamweaver I see all the text but i do not see the video window or anything appertaining to the video. Am i missing something out here, or is there other things i need to do.

I assume you followed this guide here on how to embed your form in Dreamweaver: http://www.jotform.com/help/36-Adding-Form-to-Dreamweaver, if not please do.

It is also possible that the application does not allow viewing the embedded video directly on it and you might need to preview it or publish it first. 

You could also try viewing the web page you are creating in a web browser and see if the video will be displayed there. Or you could also try embedding the iFrame code instead.
