When any person open my forms he get that the form is disabled???

i have know build jotform and even in my first he said that my form is disabled what do i did rong and here is the disabled form

Our Form Builder does not allow Login forms similar to what you have created.

This was the reason the phishing filter had already automatically suspended the form and your account.

If you need your account reinstated, we will have to delete the form first.
