Issues with form template

    Hello, I used one of your templates to create a form.  Everything looks great to the end user.  However, when I receive an email notification of the response, it shows the form within that email, and all the text on the form is from the original template (it does not reflect the changes I have made).

     

    Is there a way to correct this?

     

    Below is a link to the online form.  You can see that in the text portion, everything is in our business name (Ultimate Basketball Fieldhouse). 

     

    However, in the screenshot, you can see that it reverted back to New Berlin Pumas Select Baseball.

    I have tried recreating the situation with a copy of you cloned form but I have been unable to do so.

    It seems I'm receiving the correct email notification.

    Kindly try recreating the notification email template
