How can I post to mySQL database AND to JotForm?

I have integrated a JotForm into my page and have populated numerous fields with php, but I can't figure out how to post the data to my SQL database AND post it to JotForms.  I can do one or the other.  I'd rather not have to use another third party tool like Zapier to do this.  THanks!
