My form on six looks great yet on my iPad its gone strange

Hi I have cleared the cache on the iPad and still the form is stretched any suggestions as on the website it looks fine

I found an error in the browser console when I checked the website with the form.

Please try also correcting the issue

Then re-embed the form by following the user guide: -Adding-a-Form-to-Wix-Site

Test/check again if this resolve the issue.
