Form is cleared when submitted with unfilled fields

When a client attempts to fill out one of my forms and they are missing information, it takes them to a blank page and deletes their information altogether instead of telling them what field is missing.  I have 3 forms created through Jot Form.  The link to the forms on my site is:  http://www.discoveringstrengthpt.com/#!application/c1zzc

This does not happen with all clients, but it has happened more than once.

Please try replacing the form embed codes with the iFrame embed codes on your website.

Getting the Form iFrame Code

It should help with field validations and prevent possible redirects to incorrect pages.
