Can we create custom forms with JotForm?

    We will need a multiple screen form duplicated from an .html code that our designer built.  Below are the screens we will need and the conditions.

    Screen 1

     

    On the opening screen, if you select Yes, the form will go to screen2.  If you select No or Not Sure At This Time, the form will take you to screen3

    Screen 3

    The date here will be picked from a calendar.  The time boxes will be picked from dropdowns and the same with the time zones.

    The last screen, the fields will all be text fields.

    This is not possible to import HTML code into JotForm, so you will need to re-create the form using available JotForm tools.

    Multipage form can be created with the Page Break elements.

    Custom navigation is possible with 'Skip to a page or end of the form' conditions.

    And custom design is possible with CSS injections.

    I thought that you would be able to put this custom form together for us with the .html that I sent.  You have done it in the past.  This is a tough form and was hoping since your team did such a great job on the other one, you could do this one too.

    We do can provide assistance creating your desired form, everything from scratch using JotForm Form Builder.

    Please understand that we can only provide such extra assistance time on our spare time, since our primary task was to support users in using the Form Builder. I am saying, it may take time before any one of us can dedicate time building the form you wanted from scratch.

    I will submit your request to our support manager so that the task can be delegated accordingly.

    Has anyone had a chance to look at the form we are trying to duplicate for our home page?  We have not been able to duplicate it ourselves and would appreciate it if you could help us.

    Unfortunately, I do not see any updates as of this time.

    We have recently added a "Request a Form" option, so I would like to suggest posting a request from the My Forms page. This way our Design Team should be able to see your request.

    I went to "My Forms" page and do not see where I can request a form option.  Please advise.

    The 'request a form' option was a temporary bonus to the JotForm users. I am sorry to say, but that option is no longer available.

    You may ask your designer to design the form via JotForm or hire someone for this task (e.g. there is a freelancer marketplace - https://www.upwork.com).
