multiuser signup form

    Looking for a way to create forms for either volunteers or events where as people fill in the form previous inputs are remembered and either 'filled' blanks are not shown or are not editable. For instance an online signup sheet for reserving time to get a massage or we have an ice cream social where we need three people to stock the servers, four servers and two people to clean up. Is there anyway to create a form and send it out to many people and have the form keep track of who has reserved which spot and what blanks are still available?
