I am also interested in this. I have several fields defined by a "contact"such as name, phone number, email . I would really like to link all the fields into one group. Especially if there was a way to Duplicate all the fields on the user side to add another contact.

    Hi,

There are actually a couple ways to show sets of fields based on selections in the form or allow the user to enter another set of data.  The easiest way would be to use a widget like the matrix dynamique:

    http://widgets.jotform.com/widget/matrix_dynamique

This would allow your users to enter additional sets of data if need be.

The second way would be to use conditionally hidden form collapses with identical sets of fields.  This is a much more difficult setup.  Here is a form I helped another user create that has allows for selecting 1-4 golfers then shows the appropriate number of sets of entries.  If they select 2 golfers, 2 sets of name, address etc will be shown.

You can clone it to see the full setup:

If you have any questions about either one, let us know and we will be happy to help.

I currently use this conditional method on my form. I also use the Configurable form however this does not allow me to certify emails or set masks, and is also rather unattractive.

Separating your form into sections using form collapses, then conditionally showing the form collapses would be your best bet then.  You could setup identical sections then show them based on how many sets of data they need to enter.

The easiest way would be to setup one section, then copy then entire section using the "manage multiple fields" functionality:

    http://www.jotform.com/help/242-How-to-Manage-Multiple-Fields

This way, you only have to setup the first set of fields and make copies of the full set.  Just rename the form collapses and setup a conditions to show them.
