Conditions mixing up on their own

Hi, 

We have set up an application so that depending on the selection a person makes, they will be redirected to a specific website and their application will go to the right people. We have had trouble over the past month of our conditions mixing up so that the things are going to the wrong people or they are being redirected to the wrong link. When we get it to look at the conditions they are all mixed up. What could be causing this?

As far as I know, we do not touch users' forms. There is a chance that the issue is caused by some bug in our system, but it sounds a bit odd to me.

I would like to suggest checking a form revision history to see if there are any updates related to the form conditions.
