I am writing the privacy policy for my website. My website uses a direct link to a JotForm form, and may use an embedded form in the future.  I need to know whether anyone (other than me) is collecting personally identifiable information about my visitors' online activities over time and across different Web sites when my visitors use the JotForm forms.

    Hi,

If you are referring to the statistics of your form, you can use the Form Analytic feature of JotForm. For more information, you can check this article: Form Analytics. You can also use the google analytics widget for so you can get a more detailed information in your form. If you are referring to a different concern, please provide us more information so we can further analyze.

Do let us know if you need further assistance.
    Regards.

I am not asking how to collect this information. Instead, I am asking whether JotForm (or anyone else) is collecting this information, without my having requested that they do so, when my visitors use my forms.  My intent is to comply with California law, which requires that all websites "Disclose whether other parties may collect personally identifiable information about an individual consumer's online activities over time and across different Web sites when a consumer uses the operator's Web site or service."  You can see it here:

    http://www.leginfo.ca.gov/cgi-bin/displaycode?section=bpc&group=22001-23000&file=22575-22579

Just scroll down about a page and look for the "(6)", as in 22575.a.6.

    Hi,

If I understand your concern correctly, you want to know if other person is collecting any other information aside from the data in your form? Since we have a Form Analytics feature, we do collect  some other data from the user like the IP address, the device and platform they use in filling up the form and the the user's location (based on the IP address). Aside from those information we do not collect other information such as individual consumer's online activities over time and across different Web sites.  Once you have created a form in JotForm you have the automatic access to the Form Analytics feature. Only in your account you can view those information.

Do let us know if you have any other question.
    Regards.

I think you are saying that you collect the user's IP address even if I do not use Form Analytics. Is this correct?

Yes, you are correct. We record the IP address of the user even if you do not use the Form Analytics. The IP address can also be displayed not only in the Form Analytics but also in the form's Submission page.
