How to make form with Stripe

On another note, I am using Stripe to create a new form and want to give users the option to purchase an item, then select from one of four sizes. How can I set that up

I understand how to downgrade my plan, but I was told that I could return to the original pricing of $45 per year. However, the links that were sent don't allow me to do that so that was what prompted my earlier question. Is there someone I can communicate with to resolve this matter.
