Recently our email notifications started missing information (primarily name and phone number) that is needed to schedule new estimates for our painting business.
Setup a new email notification and that one is still missing the "Name" in the subject line, but does include Name in the body of the email.
What might cause this?
note: we did recently change to iframe embed because google analytics was showing jotfom as the 'source' for all our form submission conversions.

I was able to see the problem when I checked on the existing submission email on your form.
But when I test submitted your jotform http://www.jotform.com/form/11342801313 using its direct URL, I could not reproduce the same issue. The 'Name' field appears in the Subject Line of the submission email.

It seems that the issue was caused by the script embedded form on your website page.
I suggest you re-embed the form to your website using instead its iframe embed code.

Make sure you clean/remove first any existing script codes of the form on the website before adding it back using its iframe embed code.
Let us know if issue is not resolve even after re-embedding using iframe code.
