    Hi,

    There are several forms in your account that are against our terms of use.  The ones that resulted in your account being suspended were the "Login DriveMyCar" forms:

    We do not allow for the collection of username or password through our forms especially when they are posing as log in forms. 

    I removed the log in forms and reactivated your account. I would strongly recommend removing any fields from other forms that ask for any such data as they will result in your account being suspended once again.
